(i) Put your correct name because in some countries Google Adsense dont allow to change it after the creation of your Adsense Account. You have to put the name and spelling exact like that it would be written on your issued Government Identity Card (NIC) (OR) on your Bank Statement

(ii) Your given address should be correct and valid as Google will send you the Personal Identification Number (PIN code) on that address to verify your address and the accuracy of your contact information when your earning will reach to 10$ and make sure that the address you are going to submit is not used by any of your relatives for making Adsense account. As Google Adsense allows only one acccount for one address
